CLINICAL EXAM PROCEDURE

The candidate should submit a letter of intent, the fee of $2,000 and surgical logs of 25 dissimilar procedures of major scope and variety in which you were primary surgeon. This information must be submitted by December 31st.

After the Board has reviewed all applicant files at their February meeting, the Board Office will notify each candidate of acceptance no later than March 31st. The Board Office will also list the five (5) cases chosen by the Board from the logs submitted.

The candidate is to detail these five (5) charts and also five (5) others of the candidate's choosing, performed subsequent to the original 25 cases. These are to be brought to the examination.

  1. Each case must be accompanied by the following detailed, typed documents:

    1. Admitting note
    2. History and physical
    3. Operative report
    4. Discharge summary
    5. Summary of final outcome

  2. Each of the documents is to include the information as requested in the documentation outline on the next page.

  3. If the documents are completed properly, the examiners should be able to evaluate each case without opening the office or hospital records.

  4. However, it is necessary to bring a copy of your office and hospital records (except for the nursing notes) to verify the information included in your summaries.

  1. Admitting note

    1. Chief complaint
    2. History of chief complaint
    3. Physical findings relative to chief complaint
    4. Supportive diagnostic tests, consultations, etc.
    5. Diagnosis
    6. Plan for treatment
    7. Anticipated results

  2. History and physical

    1. Medical history

      1. past medical diagnosis
      2. previous surgeries
      3. medications
      4. allergies
      5. pertinent social history
      6. pertinent family history

    2. Pertinent general physical findings

  3. Operative report

  4. Discharge summary

    1. Describe patient's stay
    2. List of any complications and their management
    3. Disposition of any abnormal findings
    4. Condition of discharge
    5. Discharge medications
    6. Discharge instructions
    7. Follow up appointment

  5. Summary of final outcome
